Carolyn Jean Puckett, 72, of Assumption, IL died July 11, 2014 in Decatur Memorial Hospital.

Services will be 10:00 AM Tuesday, July 15, 2014 in St. Mary's Catholic Church of Assumption, with Father Don Wolford officiating.

Visitation will from 5-8 PM, Monday in Seitz Funeral Home, Assumption. Prayer service will follow at 8:00 PM.

Burial will be in St. Mary's Cemetery, Assumption.

Memorials may be made to Central A&M FFA or Assumption Community Pride Association.

Carolyn was born November 12, 1941 in Assumption, IL the daughter of Carlyle J. and Virginia Jean (Behl) Bantner. She married Carl Puckett in Assumption, August 23, 1963.

Carolyn was a member of St. Mary's Catholic Church of Assumption, she served on the St. Mary's Altar Society, was an Alderman for the City of Assumption, serving for eight years, and the Mayor of Assumption for four years. Carolyn worked for Pana Community Hospital as a receptionist.

Surviving is her husband Carl; mother Jean Bantner; son John Puckett of Fairbury, IL; daughters: Cheryl (Eric) Swigert of Stonington, IL, Chelli (Jim) Branyan, of Assumption, IL and Cara (Kevin) Reiser of Rochester, IL; ten grandchildren; brother Robert (Deanna) Bantner of Assumption, IL and sisters: Kay (Ronald) Clutter of Assumption, IL and Connie (Roy) Harrold of Clinton, IL.

Preceding her in death are her father and granddaughter Casey Williams.

The family would like to thank Dr. Esparaz & staff, Cancer Care, Decatur Memorial Hospital ICU nursing staff and Dr. Minnaganti for their wonderful care of Carolyn.
